Medical Forms for Returning Students
Students returning into Grades 3, 6, 9 need to have updated forms submitted before July 15, 2023. Forms are available on ISB’s new website here. Students who entered within the past 12 months are eligible for a waiver.
Updated Vaccine Records
Parents are encouraged to please see that they’re children are up to date on their relevant vaccines even if they are not due for a medical physical this year. Please consult your pediatrician or find up to date information here.
