Dear ISB Community,
MedPark Hospital (Rama 4, Bangkok) informed us yesterday that they will provide a limited allotment of doses for ISB Students between March 24-March 25, 2022. Please register here if interested. Official date and time of your vaccine will be sent to you once confirmed. Please be patient, we will send notices late tomorrow.
Deadline for registration is March 23, 09.00AM. This is a first come, first serve basis. Please sign up only if you will be available during the days listed above (they are tentatively in the afternoon) to allow others to use the slot.
Please find instructions for vaccination of students aged 5-under 12 years old below. Please note those who have not reached aged 5 or are 12 or over may be ineligible.
Entrance : Door 4, G floor (Side door on The PARQ side)
Document check, pick up queue and Registration : In front of Starbucks, G Floor
Vaccination : Fast Track Clinic, G Floor
Documents : 1. Student : Copy of ID card / Passport (signed copy)
- Parent : Copy of ID card / Passport (signed copy)
- Consent form (TH or EN)
- Screeningform
Please fill forms prior to arriving at the hospital to avoid delays.
Parking : Hospital parking lot (free) or The PARQ (Free 2 hours with stamp from Extension Drugstore in The PARQ)
Important Note :
- Only 1 adult is allowed to accompany the child into the area to maintain social distancing.
- Please wear your ISB Uniform, an ISB representative will be present to assist. Please arrive 30 minutes early.
- This is for first vaccines only. Students who have received other COVID-19 vaccines before are not eligible.
