ISB Health Clinic: Medications at ISB

By: ISB Health Clinic
nurse@isb.ac.th).

Many students will require medications to be given at some time during the school day. At the Health Clinic we are here to help with this.

If your child needs medication during the school day please bring it to the Health Clinic in a ziplock bag clearly labelled with your child’s name and grade/teacher. All medications should be in the original packaging with medication name and expiry date on it. Unpackaged medications will not be accepted as we need to be certain of the medication we are administering. If necessary the ISB nurse will remove the medications from the packaging and return the medications to you (for example if there is a bottle of medication).

We also need written consent from the parents to administer medications. The following forms are available on the ISB website under “Health Services”.

  1. Nurse Medication Consent Form. This form should be filled out on an annual basis (with the annual medical forms) giving the nurse consent to give you child some over the counter medications. There is an option where the nurse can either give the medication, not give the medication or call the parent every time they think the student may benefit from the medication.
  2. Parental Consent of Administration of Short Term Medications. This form should be filled in if your child is needing medications for a short period of time (such as antibiotics or antihistamines). This form will ensure the nurse knows what the medication is, what it is for and when it must be administered.
  3. Prescription Medication Consent Form. This form is for children who are on long term medications which are prescribed by a doctor. The doctor and the parents must sign this form. This form needs to be filled out again every time the the medication or the dose is changed.
  4. Field trip medication consent forms (Elementary/Middle School and High School). These forms are to be used when your child will be requiring medications whilst on an off campus trip and this medication was not written on the permission form). A new form must be filled out for each off campus trip. There are separate forms for Elementary/Middle School and High School.
  5. Anaphylaxis Treatment Plan, Diabetes Management Plan and Asthma Care Plan. These forms are to be filled out by the parents and the doctors of students with anaphylaxis, diabetes and asthma. They outline the treatment the student required for these disorders. These forms should be filled out on an annual basis as treatments and recommendations may change.

Nurses will NOT accept medications that are dropped into the clinic if they are not clearly marked, in the original packaging (and not expired) and with the appropriate consent forms filled out.

ISB is committed to your child’s overall development, both educationally and health related. Should you have any questions or concerns, please contact your healthcare provider or the ISB Health Clinic (nurse@isb.ac.th).
